Before your appointment please take a few moments to look through this information to be prepared for your child's appointment.
The Children's Center Pediatric Clinic strives to provide the best possible care for your family. To help accomplish this in the most timely manner possible, we have included the initial patient forms for your convenience online to fill out before your first appointment.
What to bring for your appointment:
- Insurance card and insurance information
- Social Security number
- Referral or authorization form, if required by your insurance
- Any recent CT scans, MRI scans, or appropriate medical records
- Current immunization record
At the time of your appointment, you will be responsible for:
- Providing current/updated insurance information
- Providing current/updated address, phone, and employment information
- Payment of any co-payment or private pay amounts due at the time service is provided
- Payment for services not covered by your insurance plan
- Ensuring that the Clinic has any authorization or referrals required by your insurance company
